What you’ll do:
With a minimum of 2 years clinical experience with a focus on managed care, and with UAS experience, you’ll be experienced in Home Care, Long-Term Care, and Managed Long Term Care, including appropriate support services in the community.

With solid assessment skills, you’ll have the ability to understand and apply coverage guidelines and benefit limitations and be familiar with clinical needs and disease processes for chronic physical and behavioral illnesses, (depression, challenging behaviors, Alzheimer’s disease and other disease-related dementias). You’ll handle all the relevant paperwork and decide on eligibility for community-based long-term care. Knowledge of Medicare and Medicaid will be beneficial.


You'll need to be comfortable visiting members in their homes, possess a valid driving license, (optional for those working in Manhattan), and confidence in other languages would be an advantage.  You’ll need to be a great organizer, a collaborative team-player and have good IT and data skills.
