Job DescriptionPrimary function is to supervise the care worker staff (care workers, senior care workers, and clinical health associates, hereafter referred to as care worker) in department of Case Management Programs who are responsible for supporting clinical team with various care coordination workflows. Provides leadership and subject matter expertise in care coordination, clinical program concepts, methods, and activities for unlicensed teams. Builds successful relationships with behavioral health providers, physical health partners, community services and agencies.
- Manages the care workers in the case management programs and supports services (when needed) to ensure compliance with policies, procedures and customer requirements.
o Measures performance and productivity against the standards utilizing workflows and performance metric measurements, and holds staff accountable to expectations.
o Ensures the staff are meeting telephonic and compliance standards for Magellan.
o Provides initial training as well as ongoing training and routine staff supervision activities regarding staffing, disciplinary actions, and performance appraisals.
o Interviews and screens applicants for care worker positions within the case management programs.
o Monitors and evaluates data tracking for each program for accuracy, completeness, and performance issues.
o Ensures care workers are provided with feedback and accountability standards through 1:1 coaching and evaluation.
o Ensures motivational interviewing technics are being utilized.
o Conducts bi-weekly call audits to ensure adherence to standards, and member satisfaction.
o Identifies training needs for the Case Management Care Worker sub-group in developing specific training programs for the care workers.
o Responsible for the training and development of lower level staff and maintaining a healthy cohesive teamwork environment.
o Manages the time sheets in workday, as well as addresses any use of unauthorized OT.
o Coordinates with Triage/Appeals and CCR teams as needed in support of case management team needs.
o Provides care worker duties as needed in support of programs whenever necessary (short staffed, etc) as defined below and to include other duties and tasks as needed:

o Provides non-clinical support to workflows involving utilization management, case management, disease management, and health promotion.
o Troubleshoots when an appropriate provider, type of service, or appropriate level of care cannot be readily found.
o Has current knowledge or researches availability of community resources and services and link members to appropriate services. May assist in transitions in care for hospitalized patients and makes appropriate placements, if needed.
o Assumes responsibility for self-development and career progression.
o Proactively seeks and participates in ongoing training (formal and informal) in all aspects of the Care Worker role.
o Assists in the mentoring and training of new Care Workers.
o Remains responsible for updating self on ever changing information to ensure accuracy when dealing with members and providers.
o Coordinates and manages distribution of correspondence and materials to enrollees and providers.
o Participates in operational activities, including data collection, tracking, and analysis.
