JOB SUMMARY
Facilitates Health Services department workflow by supporting clinical team with various care coordination workflows. Locates community services and other referral locations for enrollees, arranges access to care, supports communication with hospitals, and providers.

Essential Functions


Provides non-clinical support heath services workflows involving utilization management, case management, disease management, and health promotion. Trouble shoots for the clinical team when an appropriate provider, type of service, or appropriate level of care cannot be readily found. Researches availability of community services in order to link members to appropriate services. Assists in transitions in care for hospitalized patients and makes appropriate placements as needed.
Coordinates and manages distribution of correspondence and materials to enrollees and providers.
Participates in operational activities, including data collection, tracking, and analysis.
Assumes responsibility for self-development and career progression. Proactively seeks and participates in ongoing training (formal and informal) in all aspects of the Care Worker role. Assists in the mentoring and training of new Care Workers. Remains responsible for updating self on ever changing information to ensure accuracy when dealing with members and providers.

MINIMUM REQUIREMENTS
Education: HS/GED
Experience: 1-3yrs
Industry: Health Care or related field
Equivalent combination of experience and education

Knowledge, Skills, Abilities:
Computer Skills: Basic computer skills

PREFERRED QUALIFICATIONS
Education: Associates or Bachelors degree in a behavioral health related field preferred.
Experience: Knowledge of Excel, preferred. Experience in data entry in fast paced environment. Demonstrated ability to multi-task while maintaining accuracy.
Knowledge, Skills, Abilities: Ability to work independently and as a team member.
Other: Understanding of plan benefit structures. ICD9 and DSM coding and local communication resources; excellent verbal and written communication; able to multi-task.

Experience in a managed care environment, integrated medical and behavioral health care and/or training in chronic pain management.
  • Understanding of plan benefit structures, psychiatric/medical terminology, and local communication resources.
  • For roles supporting the Chronic Pain Management SBU, understanding of Chronic Pain Management guidelines and workflows is also required.
